Talk Abstract
Surgery for esophageal adenocarcinoma poses certain challenges. Given its pathogenesis, esophageal adenocarcinoma patients in Western countries are more likely have central obesity and gastroesophageal reflux disease. Performing a good oncological operation (adequate lymphadenectomy with clear tumour margin) in morbidly obese patients is technically challenging. In addition, those who had sleeve gastrectomy necessitate the use of alternative conduit to stomach. A bulky EA with significant extension into stomach requires careful surgical planning to balance risk of perioperative complications with good oncological outcome (extended total gastrectomy versus esophagectomy). Case discussions will be used to demonstrate some strategies (tips and tricks) used to address some of these challenges.
